LinkZorg is a digital platform designed to help emergency staff at OLVG Oost Hospital in Amsterdam connect 3U patients—those who are unhoused, undocumented, or uninsured—with essential follow-up care.
Our design team developed the platform over 12 weeks as part of the Master of Digital Design at HvA, responding directly to feedback from hospital doctors, nurses, care coordinators, and social workers.
I contributed to research, UX/UI design, and project leadership, guiding the team through critical decision points to shape a tool that’s both practical and sensitive to patient realities.
The live website features a custom resource database, multilingual filtering, and a printable care plan generator, all designed to reduce discharge stress and improve health equity.
Recognized by HvA and OLVG Oost, LinkZorg has been adopted for implementation by the hospital, where further testing will help refine the database and user experience.
